SUMMARY: The LPN CC provides clinical leadership to the household including dispensing medications, observing residents’ condition and communicating with the Physician/NP and family /responsible party as needed. The LPN CC participates in planning, organizing, directing and facilitating the clinical operations of the household to ensure regulatory compliance, nurture a person-centered and resident directed culture, and strengthen the ability of the household to grow as a separate effective home, and promoting individual and team learning in the household. Nurses are teachers and role models of professional nursing practices to household staff and resident advocates for quality clinical care. The Nurse provides each resident with professional licensed nursing observations and evaluations and coordinates their care with other household staff.

The Continuing Care team is committed to serving residents in all aspects of their daily lives and assisting with the day to day functioning of the neighborhood. Members of the Continuing Care team will develop relationships with the residents and their families using a consistent caregiver approach; supporting the residents’ physical, spiritual, emotional and clinical needs and focusing on placing the residents first.
Through daily interactions, the Continuing Care team develops firsthand knowledge about the residents’ values, needs, preferences, and life history. Members of the Continuing Care team provide hospitable care and services to the residents, treating them as individuals with strengths and gifts and actively listening to their voice in what care and services we provide.
ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

- Current CPR certification

- Complete a Safe Food Handling and Preparation Class (within 30 days of hire)
EDUCATION and/or EXPERIENCE:
College/university or licensed nursing program as required by state Board of Nursing. Related long term care/assisted living experience preferred.:
