SUMMARY: The Program Assistant III will stimulate the physical, intellectual, social, spiritual, and emotional well-being of the residents residing in Continuing Care.Assess for the resident’s needs/desires related to purposeful engagement and providing therapeutic/recreational activities to meet the daily needs and interests of the residents. Completes state required documentation.

Person Centered Care Summary:

The Continuing Care team is committed to serving residents in all aspects of their daily lives and assisting with the day to day functioning of the neighborhood.  Members of the Continuing Care team will develop relationships with the residents and their families using a consistent caregiver approach; supporting the residents’ physical, spiritual, emotional and clinical needs and focusing on placing the residents first. 

Through daily interactions, the Continuing Care team develops firsthand knowledge about the residents’ values, needs, preferences, and life history.  Members of the Continuing Care team provide hospitable care and services to the residents, treating them as individuals with strengths and gifts and actively listening to their voice in what care and services we provide.

ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.

1.Uses independent judgment to design, plan and facilitate various structured and impromptu programs to individuals and groups in common areas and resident rooms.

2.Escorts and ensures the safety of residents on group trips and excursions.

3.Works collaboratively with other team members, management, volunteers, and families to deliver programs and activities designed to enhance the resident experience.

4.Routinely monitors changes in resident’s needs, abilities, and preferences to provide programs and activities appropriate for individual and group audiences.

5.Generates program supply lists, maintains supply levels, and ensures safe operation and functionality of equipment.

6.Collects and records information by conducting interviews and reviewing documentation for assessments.

7.Consistently maintains proper documentation including attendance sheets, progress notes, assessment forms, service plans/care plans and generates the MDS documentation and assessments.

8.Participates in team discussions regarding resident programming, monthly calendars, and resident issues.

9.Participates in care and service conferences providing vital information to the resident and significant others regarding programming involvement.

10.Performs other duties as assigned.

Qualifications

QUALIFICATIONS:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. ·Understanding of the dementia process

·Independent thinker, self-motivated, organized, flexible and dependable

EDUCATION and/or EXPERIENCE:

  • High School Diploma required; Associates or Bachelor’s degree in related field preferred.

·Five (5) or more years’ experience as an Activities Assistant in a licensed health care facility is required.

·Experience leading groups of people with varying levels of physical and cognitive independence is required. Experience with the elderly preferred.

  • Flexible work schedule including weekends and evenings as appropriate is required.

SUPERVISORY RESPONSIBILITIES:
Directs the work of volunteers during programming activities.

·Strong oral and written communication skills

Must be able to lift 50 lbs. from floor to waist.Able to transport a resident in a wheelchair.Able to walk for long distances or periods of time, pushing weights up to 250 lbs.Able to kneel, bend from the waist to floor, stoop, twist torso laterally in all directions, and grasp small and large objects.
