SUMMARY: Responsible for developing and maintaining maximum census in Continuing Care while meeting or exceeding the Erickson Way Values. The Health Care Counselor presents and sells Continuing Care to prospective external and current internal residents.

All information regarding the resident is confidential. This position requires maintaining all confidential information regarding the residents.

Working Relationships:
Interfaces with residents, family members, department managers, nursing staff, resident services, outside callers, social workers, accounting, medical staff, sales and information and outside networking professionals.



ESSENTIAL DUTIES AND RESPONSIBILITIES: include the following. Other duties may be assigned.
1. Tour prospective external residents and current internal residents through the Continuing Care, offering an overview of the community, health care offerings, pricing, amenities and availability.
2. Counsels with decision-making (floor plan, timing, etc) and sells prospects entry onto the appropriate waiting list.
3. Initiates contacts with and pursues contact with standby list members with the goal of reoccupying available Continuing Care units.
4. Supervises the coordination of move-in dates; admissions, and transfer of resident throughout the continuum.
5. Develops and leads implementation of Quarterly Strategic Sales and Marketing Plan (in collaboration with Corporate Health Care Sales partners).
6. Trains, develops, and manages the Admission Team by holding weekly/monthly informational and developmental meetings to include Medicare coverage changes, Managed Care information and other pertinent information for family members considering admission to Continuing Care.
7. Attends Utilization Review, QA/QI Committee, Transitions and Operations Team Meeting.
8. Responsible for the oversight and understanding of the job responsibilities for the Health Care Associate, Move-in Coordinator, and Community Outreach Coordinator. Able to perform those responsibilities in the absence of staff or if the position is vacant.
9. Coordinates all aspects of the education and ongoing training process for the health care sales team.
10. Coordinates and supervises the outreach efforts and networking inside and outside the community.
11. Prepares and provides weekly and monthly reports for the Administrator and Executive Director .
12. Follows up with prospective residents through phone calls and written correspondence to provide additional information.
13. Attends special functions/events. Provides tours, answers questions and occupies the sales information table at these events.
14. Identifies daily admissions from multiple sources. Assesses referrals for potential admission to Continuing Care. Assists with external admissions to Continuing Care.
15. Performs care coordination to facilitate transfers from acute hospital to Continuing Care.
16. Collaborates and communicates with community health care professionals to secure approval for admissions and notify relevant parties of impending admissions.
17. Abides by HIPAA standards and by resident rights and responsibilities.

Qualifications

QUALIFICATIONS:

• Computer skills including Word, Excel application use.
• Knowledge of health care delivery systems and reimbursement mechanisms, medical practice standards, utilization review and quality assurance procedures.
• Analytical skill and business acumen.
• Interpersonal skills needed to interface with health care professionals, senior-level management, and patients/families in crisis.
• Excellent oral and written communication skills needed.

EDUCATION and/or EXPERIENCE:
• Bachelor's Degree in related field preferred.
• Minimum of 5 years experience in Continuing Care service line sales preferred.
• Experience in assessing prospective hospitalized residents to determine eligibility for skilled nursing care required.

SUPERVISORY RESPONSIBILITIES:
This position is responsible for supervising the Health Care Associate(s) and the Move-in Coordinator.

LANGUAGE SKILLS:
Must be able to read, write, understand and communicate in the English language.


LICENSES, CERTIFICATES, REGISTRATIONS:
Must have valid state driver’s license.
